jeep jerking and slow acceleration

Hello! I have a 2005 Jeep Liberty, 2.8CRD, automatic,diesel. The problem I've been having is that when the engine warms up the car is very slow to accelerate, for example at a red light or on the highway. Also, it sometimes jerks when changing gears and i try helping the car by timing the acceleration and gear change. Also, this winter the jeep died twice at the same intersection, i stopped and it rattled and died (like a manual would). This happened twiced in the span of 3 months. Is it some faulty sensor,transmission problem,dirt or what? Jeeps are very expensive to repair in my country and there aren't many good mechanics and i want to try and figure out whats wrong before i take it to a mechanic or repair it myself. Thank you for the help! Also, these are the codes that came up on OBE: P0876, P0869, P0932. Here's a pic of my jeep!

I'd start with cleaning the MAP sensor.
Is it hard to start, especially after sitting for a while?

never had any problems with starting the engine, also i checked the air filter and its fine.

That's one common problem eliminated then, they're known for air leaks in the fuel system. Has the fuel filter been changed recently? A choked filter will starve the engine.

All of the Trouble codes you listed are transmission related. The torque converter failing to release lockup and stalling the engine when you try and come to a complete stop is a known issue with the 545REF transmission especially when they get a lot of mileage or age on them. Replacing the front pump assembly, which requires removal of the transmission, will solve the stall issue. Read the attachment and it will explain the stall causes and repair in detail.
As to your P codes, this is what they are:

P0876 -- Transmission Fluid Pressure Switch rationality
Transmission Fluid Pressure Sensor/Switch D Circuit Range Performance.” It's logged when the computer detects the transmission fluid pressure sensor's voltage isn't within the normal range.

P0869 -- Transmission Fluid Pressure High
Indicates that the transmission fluid pressure in a vehicle is too high. The transmission control module (TCM) monitors the actual line pressure and the desired pressure, and if the actual pressure is consistently higher than the desired pressure, the P0869 code is stored. The difference between the two pressures is usually around 10 psi, but it can be higher

P0932 -- Hydraulic Pressure Sensor Circuit Fault
Indicates a problem with the electrical system in a vehicle's hydraulic pressure sensor. This could be caused by a number of issues, including:
  • Defective electrical components, such as short circuits, damaged wires, corroded cables, or blown fuses
  • Loss of ground
  • Faulty hydraulic pressure sensor
